Danger
Before plugging the FormBox into the mains electrical supply, check that the voltage and power
supply comply with the specications indicated on the rating plate of the appliance. You will nd
this information on the sticker on the back of the machine.
The ceramic plate of the heater on the underside of the top unit of the FormBox can become
extremely hot under operation and may cause serious burns. Do not touch under operation.
When in use, surfaces directly under the heater of the FormBox will become very hot.
When sheets are heated in the FormBox they will become very hot, do not touch Mayku sheets
until they have cooled after use.
The FormBox melts plastic sheets and other materials which may emit fumes and odours under
normal circumstances and if overheated. Always use the FormBox in a well ventilated area.
Never cover the ventilation holes on the top, sides or underneath of the FormBox.
The FormBox must not be submerged in water, as any contact with the electrical parts would
create the risk of malfunction and electric shock.
To prevent the risk of electrical shock, do not immerse the power cord, plug, or any non-
removable parts of the FormBox in water or any other liquid.
Never allow any liquid to get onto or inside of the FormBox.
Do not use the FormBox with wet hands.
Do not orientate or operate the FormBox upside down or on its side.
Disassembly of the FormBox should only be performed by a certied Mayku repair specialist.
The use of attachments, not specically recommended by the manufacturer, must not be used
as they may cause personal injury or damage to the FormBox.
Do not replace any part of the FormBox with a third-party component.
Do not use any ammable or potentially explosive materials in conjunction with the FormBox
either as sheets or templates.
Beware of crushing objects and body parts between the two FormBox trays or between the lower
tray and the base of the unit.
Do not attempt to use any body parts or living things as templates in the FormBox.
